Symptoms of the issue:
You have attempt to enable an instance for LA 365 through Dynamics but when you submit are presented with this error:
This error occurs is due to an issue that affects some instances where the instance has two versions of 'Live Assist for Dynamics 365 Powered by CafeX' app available in their 'Manage Applications' tab.
How to Fix the issue:
The way to fix this problem is to simply return to your 'Manage Applications' page and click the other link to which you clicked to produce this error, below is an example of two applications in one list:
